[FORM] Birthday drawing (Patreon)

As you might know, for all tiers there is the benefit of a birthday drawing.

To claim it, please fill in the form, so I can know which month you'd like it, as well as which character you'd use.

For privacy reasons I only ask for the month, but if you want to tell me the exact day I'll send the drawing the day before! If not, at the start of the month ^^
