Interesting point that you touched on a little here. The infirmary is the medical office where Sara is generally seen, it's part of Michael's escape plan because it has a window leading to the outside. The medbay seems to be a more general treatment area within the prison used for broader medical care.

Not sure if you've played any of the classic Mortal Kombat games, but John Turk who played the inmate Turk played The unmasked Sub-Zero & the masked male ninjas in Mortal Kombat 3 & Ultimate Mortal Kombat 3 For this 2 parter, starting a riot, while necessary, was pretty risky because of the chaos. Things could've gotten a lot worse then just T-Bag & a rookie guard seeing the tunnel entrance. Michael going to save Sarah too, while a morally good option, was even more risky too as it comes really close to exposing his plan.

I think the problem with this path to the infirmary is that it involves them going on the roof, where they might be more visible than going through the pipes. I think they are trying to spend the least amount of time out in the open.
