It's hinted that she was already pregnant, and that the creatures can rapidly absorb nutrients from their hosts. Just a theory, but guessing based on Fayes' size vs the other woman, they'll gestate as long as there's food available and "birth" themselves if the host can't provide for them.

That plays into my personal fan theory that the first stage of the creatures’ life cycle was microbial and the starfish are what you get after the first round of “hatching”. This also means that something even more different could be inside Fayes.

I knew she was pregnant (though how that happened is another mystery) but from re-reading the first chapter I figured she got bigger when the suppressant gel hit the entire room and was probably unconscious and the alien decided to feed her.

Its actually interesting how this can happen in real life. Some viruses can insert their genetic material into a host cell, but not lyse the cell (Burst) immediately, unless the host cell is put under a survival pressure (Not enough food in this case)
