It has been mentioned before but just to bring some clarity into it again because the translations make it confusing as the words aren't as distinct as in Japanese. Anshi is the current Empress Dowager, the Emperors mother. and is usually referred to as such. The previous Emperors mother, the previous Empress Dowager, is regularly refer to as just Empress in the subs, this is meant as indication that she held actual power as sovereign through her son, the LN translations go as far as to call her Empress Regnant. The official positions are Empress Dowager "Kōtaigō" and Empress Consort "Kōgō", but the mother of the previous emperor is called Empress (Regnant) "Jyotei". That's also where the "so-called" comes from, there is no Empress (Regnant) in the Li Emprie (the fictional country this is set in), that position doesn't exist but people still refer to her as such, because against all traditions she actually held political power befitting that position.

We're slowly stepping into some dark territory of Apothecary Diaries. Good discussion points, no notes. I suggest also that when theory crafting, don't just try to solve a "universal truth" but think from the perspective of the characters and what THEY know, could know, chose not to know, or been instructed to believe in, since all the characters are unreliable narrators with missing information that we the viewers have privy to from a more omniscient presence. :D And All I can say is to not be shy from trying to cook with missing ingredients, and guessing things!
