It's impossible to get used to the things Subaru has to live through, they are the worst things a human could ever experience of physical and especially mental trauma , while some other things he lived through were not even meant for humans but he experienced them nonetheless because of his return by death, he can't just "get over it" . He's also a very impulsive person too by nature, it's not that he can't think with a certain levelheadedness but not when he's experiencing suffering beyond what a human can think of.

Also about Emilia's death my interpretation is that Subaru really got fed up with all of this and intended to tell Emilia no matter what, like literally he suffered so much that even the warning and fear of the witch was not able to stop him anymore as a result Emilia died, probably the witch's doing.

For context of the last four episodes Subaru kept thinking about himself as Emilia's undeniable "savior" despite being utterly powerless and because of that he was literally wanting for something bad to happen just so he could "fix it" and prove to Emilia that she needs to depend on him. That's why he smiled every time someone mentioned that Emilia herself or her position as a candidate was going to be endangered by the presence of the witches cult. And that's also why when he sees the huge pile of corpses and that Rem was dead he says: "This isn't what I wanted" (admitting to himself that he wanted something terrible to happen but now that his wish came true he just sees how horrible he was for wanting that).
