According to Mike Pondsmith, the creator of Cyberpunk, Adam Smasher is a cyberpsycho, just a high-functioning cyberpsycho. He was born a psychopath, so he wasn't sane from the get-go, and he has a job that already allows him to kill anyone he wants. One thing you talked about earlier in the show is therapy, which does exist and helps just as much as downgrading in terms of holding off cyberpsychosis; the problem is that it's twice as expensive as the most expensive cyberware.
