"How have they not heard any of this?" It's a huge, top secret government medical facility with multiple floors in multiple wings, not just someone's barn. And Homelander's super hearing has been shown to only work when he focuses, and he's currently incredibly distracted, and infected with aggression.

i can understand not hearing what theyre saying but them fighting and butcher throwing kimiko against the wall. i feel like he definitely should've heard that

quinns power was basically to make everything in the area angry i suppose, releasing the spores of whatever chemical. Also this episode is sort of setup for the upcoming prequel which will dive into soldier boys backstory and old vought. And about where the V1 is at, butcher and them came across an empty cabinet where it seemed like it would've been and they hypothesized that someone else from the original trials would have it, specifically a guy named bombsight.

The facility was built to test torture on potential Supes and was redacted from satellite imagery. I'd suspect it would be rather soundproofed, especially knowing there is a Supe whose power is just a lot of decibels. And again, it has multiple wings with multiple floors, some underground.
