Contact info form for 100 Patrons Giveaway (Patreon)

Hi folks! This post is a little info-heavy, so bear with me please! 

So, the time has come for you to start entering your information and, for those on the $5 and $10 tiers to let me know what they'd like me to do for their hand-written stories! Please do that by clicking the link to the form. 

Due to the fact that this is a huge project for one person to do alone, it will take me a while to get through them all, so please be patient. On Wednesday I'm also going away on a walking holiday that we booked way before I announced this giveaway. I'll work on the 5/10-sentence stories while I'm away for you, but I won't start posting them out until June. Plus, the stickers are still on their way to me and I want to order some more once I know these ones are ok!

This form will be open until the end of May, and will close to everyone at midnight on 31st May, GMT. That gives you (including today) thirteen days! 

Finally, those who would prefer just to receive a digital story, and nothing in the post, you can now get in touch (preferably on Patreon, but I'm also available on Tumblr since Patreon isn’t the easiest to navigate at times...) and request your 5 or 10 sentence story. Please include your patreon name, tier, and the details you'd like for the story (reader/character gender, monster type, and n/sfw).
