Yep, it can happen. And no, it doesn’t always mean arousal or attraction. Spontaneous ejaculation is a real (and rare) body response that can pop up in non-sexy situations like during pelvic floor therapy. This can happen whether you have a vulva or a penis.

Sometimes it’s linked to stress, nervous system quirks, or just biology doing its own thing. Bodies don’t always follow the script we expect, and that can feel confusing or embarrassing in the moment.

In this video, I break down the science behind ejaculation, why it can happen when you least expect it, and how pelvic floor therapists should respond to these "unintentional" situations.

Very informative. Thank you!!

For me the possibility of an unintentional erection is easier to understand than ejaculation. That would really be awkward. Having J in the room during treatment is as much to make sure the therapist knows what the erection or ejaculation "means" as to give me support. She can be confident I'm not going to get funny with my wife or other chaperone in the room and that makes me feel more comfortable. In thoughts about these situations, I might prefer that the therapist continues with treatment with the erection if it is not in the way. In the case of ejaculation give me something to clean up, but remain. The thought is that her leaving the room might underscore the situation as being embarrassing while her working around it would go further to normalizing the reaction. What do you think?

If that was to happen, there’s nothing to gain in vacating the room. It’d make it feel like a big deal when it shouldn’t be. Towel off and proceed on. When I get a dental cleaning, I’m subconscious about stopping and clearing out before I start gagging sometimes, so I’ll look for that proper moment to STOP, lol, just have to communicate.

Being able to get an erection would be great !!!

I loved the "bonk" sound, when the spinal column struck your head! You continue to crack me up!

Dr. Susie Gronski - Doctor of Physical Therapy

I think you and Doug have made valid points about leaving the room. I think it's best to always ask the patient what they would prefer; some would like a moment to regroup while others don't. I air on the side of asking before leaving.

So many medical providers would benefit by learning client relationship from PFT.
