Today’s video covers two questions that were essentially asking the same thing: weak or incomplete ejaculation, muted ejaculation sensation, and what might be causing sudden changes in ejaculatory volume or force. I also talk about retrograde ejaculation, medications, aging, pelvic floor involvement, and all the possible contributors that can play into this experience.

Welcome to the first video in this month’s Ask Dr. Susie G Q&A series. At the time of this recording, the poll was a 50/50 split on whether you wanted one long video or separate shorter ones… so I made the executive call to try something new. For the next four days, you’ll get one question and one answer delivered to you daily.

Give it a watch and let me know:

Do you like the shorter, separate-video format?

Dr Susie. Could pundendal neuralgia also be a cause for loss sensation.

John Bastian

Wow, this could create a hundred questions or statements from us men that have had Prostate surgery.

The mind also plays a huge part, as I learned while having to make an effort to be sexually active the six months I was castrate. With low testosterone, more of the drive can be mental, making it more difficult to focus to the high level of arousal for a strong orgasm. I've been on testosterone replacement for two years now with none of the supplements that help keep the testicles alive so my ejaculate is clear. Not sure what parts of my body are contributing to the ejaculate.

There are as many answers as there are individuals, because nobody’s the same. What worked for me was nutrition: finding what was missing and making sure I was getting it.

Excellent point, Guy! There are urethral glands that produce secretions and testosterone therapy doesn't completely eliminate all fluid production - might be less but still some produced.

Thanks Dr. S. My issue lately has been when I ejaculate, I have some discomfort and warmth from anus, through the perianal, penis and ejaculation. Is this one of the signs of prostatitis?

Eric Hegeman

On the topic of testosterone treatment causing it, my volume appears down significantly since starting treatment. My semen also seems much thinner and more clear and less white. Could it be that I'm producing the close to the same amount of fluid, but because it's thinner in consistency it seems like less? And could the testosterone therapy be the cause of that?

Eric Hegeman

And on the topic of the format, I'm good either way. On the poll I had voted for one long one but with the camera I had put in the comments on the poll

Dr Susie Can internal massage help with weak and loss of sensation with ejaculation

Dr. Susie Gronski - Doctor of Physical Therapy

Hi Fred, It could be signs of prostatitis but it could also be pelvic floor issues. Best way to know if you're dealing with prostatitis is with a Meares-Stamey test (a test that involves a medical prostate massage followed by bacteria culture analysis to asses for infectious cells).

Eric Hegeman

That's what I figured, especially with the timing of it
