Hey friends, welcome to Day 3 of the Ask Dr. Susie G series.

Today’s video answers a member's question about testicular pain that shows up with walking and bending. If you want to learn more about the relationship between the low back and testicle pain, watch this video and this one.

In the video, I break down the key anatomy involved like nerves, muscles, and the support structures that connect your scrotum to the rest of your body. We talk about how areas like the lower back, abdomen and pelvic floor can all refer pain to the testicles during walking and bending movements.

I also walk you through a few practical strategies to explore musculoskeletal contributors. Simple things like gentle mobility and load management to gradually increase tissue tolerance and function.

If you’ve been dealing with this kind of discomfort, I hope this gives you a clearer understanding and a starting point for relief.

Thanks Dr. Susie. I guess my issue is internal testicular cysts. And the fact I’m naked most of the day. I will try your stretching and see how that goes. I’ve been incorporating massage in these areas and it seems to have loosen up the pelvic muscles. Thanks for all that you do. Keep the videos coming. ☺️

You're welcome, Bob! And if you are naked most of the day, it might be helpful to give your balls a little break from the hanging without support...I had a patient with testicular pain and their pain resolved wearing gently supporting underwear.

Bob L

But I like being naked. 🥺

You’ll be happy to know that my balls have been supported since 1:30 pm. 😁

Dr Susie. I have a form of pudendal neuralgia and this also causes the testicles to hurt. Sometimes even putting on underwear bothers me. I also get the discomfort when I sit.

Dr. Susie Gronski - Doctor of Physical Therapy

It sounds like the posterior scrotal branches of the pudendal nerve might be involved here. Gentle scrotal mobility work and nerve-friendly movements can help calm things down and improve tolerance over time. The key is slow, comfortable, and never forcing it. Check in with your provider or pelvic floor therapist so you can get guidance that’s specific to your body and symptoms. You don’t have to figure this out alone 🙂

Thanks Dr Susie. We have been working on a lot of breathing exercises and some gentle stretch’s but I usually have more discomfort the next day but then it feels a little better.

I’m getting a big break now as getting ready to board flights that will take me to Ireland. That’s where my late wife was from. I’ve still been going over to stay with here folks for Christmas since Clíona passed in 2021.
