Sorry for long comment, but I have 10 years experience working as a clinician in dementia diagnostics. Biden has Lewy body dementia. Stiffness/rigidity (watch him walk down stairs with stiff legs after Jill’s post debate speech, or not be able to raise his arms past his shoulders, slow to turn his neck, face doesn’t move/droops unilaterally although that could be Botox) concentration deficit (trailing off mid thought, walking off stage) blank expression, irritability/reactivity, it’s on the spectrum of Parkinson’s as Liz mentions, but lacks tremors at this stage. His cognitive deficits started before the physical (I actually noticed he was different in 2019) so that’s considered LBD rather than Parkinsons dementia. His tremors will probably start in a few years. He’ll probably be dead within 7. Somewhat easy to identify if you are in the field. He’s a particularly clear case, and I would say he’s in the middle stages. He probably can’t write anymore. Has anyone seen footage of him signing something recently? I think there’s been a profound decline recently— after 2-3 years the standard LBD drug cocktail (L-dopa, aricept, memantine, SSRI) stops helping. The atrophy outpaces the chemicals these drugs are able to pump out of the remaining cells. So, If he had 500 neurons when he started these drugs, but now has 50, no matter how many medications he takes the 50 neurons will never produce what the 500 did. We will see a precipitous drop in his capacity at this point. In his “energetic speech” the day after the debate, he seemed like he had pressed speech and I wondered if they shot him up with some kind of stimulant. But who the hell knows what he has access to. Anyway that’s the op ed no one will write, including me.

I don’t think we can tell if trump is declining abnormally into a dementia AT THIS MOMENT. Biden is. Trump is aging like a male with lifelong, severe ADHD/some dysexecutive syndrome and he clearly has some kind of personality disorder. There was this one time I wondered if he was having some paraphasias (unable to use precise language, but able to communicate somewhat using circumlocution), but then I never saw that again, and those with his phenotype of ADHD can jumble their speech because they’re talking faster than they are thinking.

Not for nothing - my sister is a neurologist and says she and every other neurologist she knows have all noticed he has Parkinson’s for a while and the White House is refusing to admit it. Specifically she pointed out masked facies (his blank neutral expression all the time), bradyphrenia (his delayed thinking and speaking), hypophonia (his quiet, weak, breathy speech), and the rigidity in his arms and legs. Apparently nearly 80% of Parkinson’s patients develop dementia as they continue to decline so ….

I was trying to be funny by having an incredibly specific gripe, but I guess that didn't come through. I'm not right wing. I hate Trump, I hate Biden less but there's no good option here. I'm not American, so like, I'm used to viewing US politics through ironic detachment. When I say I would be mildly tempted to vote for Trump because he is a felon, I say that in jest, but also that it would take my odds from 0% to like 1.6%, in a situation that doesn't matter because I can't vote for anyone. That's the joke. I genuinely resent the endlessly discursive poll analysis, which to be clear, is mostly a liberal, not leftist, pass time. But I do find it frustrating when I see leftists getting caught up in it because I feel like as a whole we should know better than to get wrapped up in the DNC fantasy football mindset

Im a neuropsychologist working in dementia diagnostics. I say this to emphasize that he’s been impaired for a LONG time. I noticed in 2019. I can see why a general neuro would say Parkinson’s but a specialist would probably say Lewy body.
