Do you incorporate hex crawl/survival systems into your game like that are found in the Forbidden Lands system by Free League?

Do your players explicitly tell you specific things they would like to see in future sessions (like more of a favorite NPC, or some type of spell or magic item) or do you just infer it from their reactions? Or both?
