Maomao is questioning if imperial brother is really previous emperor’s son, not if he’s emperor dowager’s son. So it’s not like she couldn’t give birth at 30, the question is if that freak is able to have a baby with a 30 yo woman. If he prefers children maybe he can’t even do it with adults🫩 at least this is how I understand it

To keep maomao's heritage simple - she knows Luomen is Lakan's Uncle (S1 Ep 24 conversation with Jinshi). She knows she is technically of noble birth, but she doesn't consider it because of her upbringing and she considers herself as Luomen's daughter only.
