I don't fully blame Meadow for just crashing out. Jackie Jr. meant a lot to her and she did genuinely love the guy. On top of losing him she has a solid gut feeling that Tony's organization had something to do with his death, which has her subconcious putting together that her own father played a role in Jackie Jr.'s death. I'd put this under Pussy's death in terms of how it's just a lose-lose situation. No one wanted to kill Jackie Jr, except maybe Furio, but they had to act out of necessity. Loose cannons are as dangerous as rats.
