Technically the mediterranean regions have onsens basically. They are called thermal baths, tons of the rich Roman patricians' summer resorts were built in those types of locations. AFAIK Pompeii had those as well, since they generally utilize volcanic vents of some kind for the heat (and these types of regions are generally where thermal baths are a thing in Europe)
