The special game was the players killing each other. It happened in season 1 also. 001 only knew what Gi Hun was planning at the last minute. One of the workers tells the Doctor in S1 while in the basement. I'm guessing the game runners are very good at predicting human behavior and can pull strings to provoke them, like limiting food.
