The concubines are essentially the Emperor's wives (they are the ones producing the kids) - there are hundreds in the harem but he only spends time with some of them (mostly the high ranking ones) - most come from noble families and are given to the Emperor to seal diplomatic relations between areas or families. Remember the woman in Episode 3 who pretended to sleep walk - she failed on purpose during her dance in front of the Emperor so he would ignore her and she would remain pure. The concubines who produce children are higher status and the one who produces an actual heir will become the Empress. Right now our girl, Gyokuyou is the highest ranked because she produced the princess Lingli, although she would be second if Lihua's male baby hadn't died. It shows how kind she is that she went to try to warn Lihua about the poison powder even though the prince surviving would have possibly made Lihua the Empress.
