The confusion is very valid but there is actually a reaction channel who theorised this entire plot twist before s2 had even came out (they were anime only). That Geto was not actually Geto, and was in fact Kamo Noritoshi. It was mostly based on the way they show Kamo Noritoshi in s1, specifically how they don't show his forehead, and how this Geto doesn't act the same as the original. So there are small hints throughout jjk that do foreshadow this. Kamo created the curse wombs, by making a woman breed with a curse spirit and adding his blood, which resulted in Choso and his two brothers that Yuji and Nobara kill at the end of s1. There are more, but Choso is the only real significant one. Choso is essentially half human half curse. One of the perks of this is that he doesn't run out of blood, which is awfully convenient for his technique. But Choso's blood mixed with Yuji and from that he could tell, due to his technique, that they were blood related. This is why he kind of loses it before he kills Yuji.
