Apparently Subaru, with the help from Roswaal and others, helped develop this special wheelchair for Rem, with cushions and belts to keep her from moving around too much and keep her comfortable. The barkeep said that it's been 400 years since anyone has able to go to the tower. From previous seasons we know that the Witches were all alive 400 years ago, so it has to be connected in some way to the Witches, maybe it's something to do with Satella herself directly. Also during the time that Ram is looking around with clairvoyance, Meili seemingly notices something "to the north". A mabeast perhaps? What crazy deaths these were, it's hard to even know what happened. It's like he got sniped out of nowhere. That snipe attack was basically the speed of light, that's why Subaru was so surprised after he died the first time. I'm glad Emilia and Rem were trusting enough of Subaru to listen to him both times. When Subaru died with Beatrice crying over him, in the source material he apparently used his Unseen Hand to wipe her tears in his last moments, as he himself couldn't move anymore. They sadly had to cut that out to make the episode pacing better I guess. This was still really amazing and sad though. As much as I'd want Subaru not to suffer, it's interesting to see him die and overcome that outcome. Crazy to see him die twice in one episode after not dying (mostly) in season 3 though. From what I've understood based on what other people have said, Subaru covered Beatrice's mouth to keep her quiet because him using Unseen Hand out of nowhere might frighten her enough for her to make sound, which could've alerted the Oiran Bear mabeasts. It's clear to see that Ram is bleeding from her eye later in the episode because whoever's eyes she's looking through (that was focused on Subaru) seemingly is just that powerful that it's that taxing on her body. It's also amazing to see Emilia and Julius both be able to react to something so fast. It's also worth mentioning for clarity that the purple stuff they fell into is supposed to be the Witch's miasma that was said to surround the tower.
